Portable Crib Child Deaths

A child has died in one of every 2,000 Playskool Travel-lite portable cribs sold. Between 1990 and 1997, more than 1.5 million portable cribs marketed by Kolcraft, Evenflo, Draco, Century Products and Baby Trend had a comparable defective device - the center hinge on the top rails. Top rail portable cribs have claimed the lives of more than 15 children.

Sulzer Hip Replacement

In the U.S., there are 1,850 people, mostly elderly, who had faulty hip replacements made by Sulzer Orthopedics, Inc.

His hip was replaced June 13, 2000, while he was looking forward to leading his daughter up the aisle in her September wedding.  He had to walk the aisle limping with a cane and “halfway through the father-daughter dance, I had to quit.”
Gretel Quick (age: 55)
Blythe, CA
Her April 2000 hip surgery left her in so much pain, she said, she could hardly climb out of bed.  “It’s been sheer hell.  I can’t work, walk, or function normally, and emotionally I’m a mess.”
Helen Simon
San Francisco, CA
A hearing and speech scientist, Simon said that learning her hip was on the recall list made her feel like a “human Firestone tire.”  She had received her artificial hip on October 16, 2000, while Sulzer was "studying” the problem.
Rhonda Silva (age: 48)
Oakland, CA
Immobilized for seven months by her defective artificial hip, Silva had the replacement itself replaced in October 2000, only to learn in December that her new hip was also defective.  “To get out of bed is the most excruciating” part of her day, she said.  “I feel like I have an alien living in my body.  I just want my life back.”


Nursing Home Abuses

A dramatic report prepared by the House Committee on Government Reform for Rep. Henry A. Waxman (D-CA) found that between January 1999 and January 2001 more than thirty percent of the nursing homes in the United States--5,283 nursing homes--were cited for an abuse violation that had the potential to cause harm. That is almost one out of every three nursing homes, and many of these abuse violations were discovered only after the filing of a formal complaint.

Nursing home failed to prevent pressure sores, falls, dehydration, malnutrition, protect Fegans from mental and physical abuse, train its staff. Fegans, who had dementia and diabetes, lost weight and became dehydrated. He developed pressure sores, hypothermia and pneumonia, which contributed to his death Sept. 24, 1999.

Nursing home failed to prevent falls, monitor and recognize changes in health, maintain accurate medical records, prepare resident care plan, notify family of changes in health, supervise and train staff. After repeated falls, Cooper was improperly restrained in a wheelchair, which contributed to his December 4, 1997 death of a ruptured aortic aneurysm.
Edna Harper

Lakeland, FL
Nursing home failed to provide Harper a wheelchair and walker, prevent a fall that led to a hip fracture, which required hospitalization. Harper died on November 20, 1997, after choking on food while a patient at Lakeland Regional Medical Center.
Edwin Jennings
Spectrum Health Kent Community Campus
Grand Rapids, MI
Nursing home failed to explain bruises and bleeding gashes on Jennings nor his missing clothing. Jennings died on March 3, 2005, under suspicious circumstances. Formal autopsy does not reveal cause of death and appears falsified, second independent autopsy reveals missing brain. Case remains unexplained and unresolved.

Nursing home failed to prevent falls, weight loss, malnutrition, provide adequate movement to prevent atrophy, provide skin hygiene, provide exercise therapy, evaluate, monitor and report McDuffie's use of sedatives.

Ethel Randolph

Nursing home failed to protect Randolph from falls and fractures, prevent malnutrition, dehydration and pressure sores, maintain medical records, keep family notified of condition, properly supervise and train staff.

Juanita Scott

Nursing home failed to prevent bedsores and infection, prevent malnutrition, dehydration, injuries and declining health. While a resident of North Lakeland Senior Living, an assisted living facility, Scott fell, fractured her wrist and developed other problems that required her transfer to Meadowview, where she developed bedsores, fecal impactions, urinary tract infections, malnutrition and dehydration. She died in January 29 of Alzheimer's Disease.

Nursing home failed to provide adequate nutrition, hydration, treat and prevent bedsores, prevent infection, maintain medical records, supervise and train staff. Huchingson suffered many injuries and unexplained bruises, bedsores, urinary tract infection, dehydration and aspiration pneumonia.
William Paul Brown

Lakeland, FL
Nursing home failed to protect Brown from malnutrition, dehydration and infection, provide adequate therapy and rehabilitation, inform resident and family of medical condition and treatment, maintain accurate medical records, administer proper medications. Brown, who had dementia and cancer of the larynx, was a victim of excessive restraints, malnutrition and weight loss, contributing to his death.


Ford Explorer/Firestone Tires
